Catastrophic injuries — those that result in permanent, life-altering disability — demand the most rigorous and medically sophisticated life care planning. When a person sustains a severe spinal cord injury, traumatic brain injury, or major polytrauma, the projected cost of their lifetime medical care can reach into the millions of dollars. Getting that projection right requires clinical expertise, methodological precision, and a thorough understanding of both medicine and the legal context in which the plan will be used.

What Makes an Injury “Catastrophic”?

In the life care planning and legal context, a catastrophic injury is typically one that results in permanent total disability or severe functional impairment, long-term or lifetime dependency on medical care and attendant services, significantly altered life expectancy, permanent cognitive, neurological, or physical impairment, and substantial future medical costs — typically in excess of $500,000 to $1,000,000.

Common catastrophic injury categories in life care planning include:

Individuals with spinal cord injury have significantly elevated rates of urinary tract infections, pressure ulcers, cardiovascular disease, and respiratory complications. Traumatic brain injury survivors have elevated rates of seizure disorders, cognitive decline, and psychiatric conditions. A physician life care planner accounts for these secondary conditions in the plan — non-physician planners may miss these entirely.

Spinal Cord Injury Life Care Plans

SCI life care plans are among the most complex and costly. Key components include attendant care ranging from partial-day to 24-hour care depending on level of injury, power and manual wheelchairs with appropriate accessories, home modifications including roll-in shower and ceiling lift, respiratory management for ventilator-dependent injuries, bladder and bowel management supplies and specialist care, skin care including pressure-relieving equipment, and specialist visits including urology, physiatry, pulmonology, and neurology. SCI life care plans typically range from $1 million to over $5 million in present-day lifetime costs.

Traumatic Brain Injury Life Care Plans

TBI life care plans are highly variable depending on injury severity. Key components include cognitive rehabilitation and neuropsychological testing, behavioral health services including psychiatry and psychology, neurologist care and seizure monitoring, attendant care and supervision for moderate to severe TBI, vocational rehabilitation for working-age adults, and medications including anticonvulsants and psychotropics.

Pediatric Catastrophic Injury Plans

Life care plans for children require additional consideration because the lifetime cost is calculated over a much longer span (potentially 60-80+ years), developmental needs must be incorporated including pediatric therapy and special education, and physician planners must be familiar with pediatric standards of care. Pediatric catastrophic injury plans frequently involve the largest projected lifetime costs of any life care planning category.
